
OFA PROCEDURE
To Our Valued Clients:
In an effort to streamline the OFA process for everyone, we have prepared the following that outlines what is needed to prepare for an appointment to submit radiographs to OFA for evaluation.
When coming to your scheduled appointment, please be prepared to have the following:
- Vaccine history if we have not previously examined the animal
- AKC or other database registry certificate (appointment will be cancelled if it is not available).
- Credit Card (Visa or Mastercard) information for the correct OFA database fee(s) - a separate form must be completed as well
- An owner or authorized signature on the submission form
- Separate payment to the Veterinary Medical Center for obtaining the images
Please note that there are multiple OFA registries, hips being the most common. Dogs must be over 24 months of age for the OFA hip registry. Less than 24 months may be submitted for preliminary OFA hip evaluation for a lesser fee--- see fee schedule.
